  • Initiation Report Form – This form is a summary to be completed and submitted at least 2 weeks prior to initiation ceremony (dues cover a membership certificate and pin.
  • New Initiate Certificate Report – Input your list of new initiate names into this excel spreadsheet template. Please list your University and Chapter Name first.  Include the first name, middle name, last name, greek affiliation, email address, classification and date of initiation. It is vital that you ensure accuracy for the spelling of your initiates name! This information will be printed directly on the certificate.
  • Chapter Officer & Advisor Update – to be submitted to Headquarters after officer transition and/or there’s a new chapter advisor.
  • Merchandise Order Form – to be printed out and submitted to Headquarters to order merchandise and pay via check or money order (to use credit card/Paypal, use our Omega Store)
  • MemberPlanet Advisor Guide – guide for chapter advisors to help transition registration of new members system from form to online portal.
